Job summary

Stone River Recovery Center in San Antonio, TX is seeking a Registered Nurse to provide comprehensive nursing care on-site. The role includes assessing physical and behavioral health needs, developing and implementing care plans, and educating clients and families about medications and conditions.

Responsibilities include administering medications, documenting care, coordinating with the treatment team, and ensuring safe hand-offs during shifts.

Qualifications

• Graduation from an approved/accredited school of nursing.
• One or more years’ nursing experience within clinical area of service provided by facility preferred.
• Experience in behavioral health/psychiatric/addiction recovery environment preferred.
• Knowledge of and experience with psychotropic medications and medication administration procedures.
• Valid RN license in the state or a compact state.
• CPR and de-escalation and restraint certification required.

Responsibilities

• Assess clients’ physical and behavioral health needs and develop care plans.
• Implement nursing care plans and maintain medical records.
• Educate clients and families about medications and conditions.
• Administer medications accurately with two identifiers.
• Notify providers of significant changes in status.
• Document effects of PRN medications.
• Ensure medication security including narcotics.
• Coordinate hand-offs and communicate with the treatment team.
• Assist with admission and discharge processes.
