Job summary

ViaQuest Psychiatric & Behavioral Solutions in a long-term care facility is seeking a dedicated RN or LPN for a full-time daytime role (Mon-Fri, 9am-5pm). You will assess residents' mental, behavioral and physical health needs, perform in-person nursing assessments, review medications, and coordinate care with the treatment team.

This position requires travel within the service area, empathy, flexibility, and a commitment to improving quality of life for residents with psychiatric needs.

Qualifications

• Graduate of an accredited nursing program.
• LPN or RN license.
• Experience in psychiatric, mental and behavioral health services.
• Willingness to travel throughout assigned service area.
• Preferably experience in LTC facilities and Behavioral Health.
• CPR certification or training provided during onboarding.

Responsibilities

• Address the mental, behavioral and physical health needs of clients on a weekly/biweekly and/or monthly basis.
• Complete in-person nursing assessments and medication/chart reviews in LTC facilities.
• Educate clients and caregivers on prescribed psychiatric and physical health medications and monitor for side effects.
• Care management: coordinate community services and communicate with the treatment team including physicians and pharmacies.
• Assist clients in building coping skills and creating annual goals related to their psychiatric diagnosis.
• Assist clinic and residential needs for assigned locations.
• Help train and mentor peers.
• May cover more than one facility as needed.
