Job summary

Community Access, Inc. is seeking a Psychiatric Nurse Practitioner to support participants and teams across the Housing Division.

You will conduct psychiatric assessments, prescribe medications, and monitor treatment to promote recovery and self-determination. You will coordinate with primary care, therapists, and support staff to ensure integrated, person-centered care within a culturally competent framework.

Qualifications

• Have an understanding, appreciation, and commitment to the philosophy and mission of Community Access.
• Commitment to person-centered treatment strategies, upholding participants’ rights, and self-determination in service provision.
• Current license in NYS as a Psychiatric Nurse Practitioner; NYS license as a Registered Nurse; Master’s degree in advanced nursing from an accredited Nurse Practitioner Program.
• Minimum of three (3) years working with people with serious mental health conditions, co-occurring substance use disorders, and homelessness.
• Minimum of two (2) year working as a nurse practitioner, including prescribing preferred.
• Computer proficient
• Excellent written, verbal and interpersonal communication skills.
• Must be fingerprinted and cleared by the New York State Justice Center.

Responsibilities

• The PNP will support participants and staff across programs within the Housing Division.
• Provide psychiatric assessment, prescribing, and medication monitoring.
• Coordinate care with primary care physicians, therapists and support workers.
• Collaborate with program teams to ensure care is delivered with self-determination, recovery, and cultural competency.
