Job summary

Community Access in New York City seeks a Psychiatric Nurse Practitioner to support participants across the Housing Division. The role focuses on psychiatric assessments, prescribing, and medication monitoring within a recovery-oriented, person-centered framework.

The PNP will coordinate care with primary care and mental health providers, document in AWARDS, and work with program teams to adapt to changing participant needs while honoring self-determination and cultural competency.

Qualifications

• Current NYS licensure as a Psychiatric Nurse Practitioner is required.
• NY prelims: NYS license as a Registered Nurse is required.
• Master’s degree in advanced nursing from an accredited NP program is required.
• Minimum of three (3) years working with people with serious mental health conditions, co-occurring substance use disorders, and homelessness.
• Minimum of two (2) years working as a nurse practitioner, including prescribing, preferred.

Responsibilities

• Provide psychiatric assessment, prescribing, and medication monitoring.
• Coordinate care with primary care physicians, therapists and support workers, internal and external to the organization.
• Collaborate with program teams and other medical providers to ensure coordinated care within self-determination, recovery, and cultural competency.
