PURPOSE:
The Adult ACT program provides an integrated set of evidenced based treatment, rehabilitation, case-management and support services to adults residing in the community who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches. Through a mobile, multi-disciplinary team of staff, recipients are supported in their recovery through an individualized, person-centered model of care.
POSITION OVERVIEW:
The Psychiatric Nurse Practitioner works as part of a multi-disciplinary team to provide treatment and support services to adults living in the community. The Psychiatric Nurse Practitioner is responsible for providing psychiatric services which includes the following: provision of a full behavioral health (including medical health) assessment; diagnosis; treatment recommendations; treatment planning; clinical treatment; and follow-up planning. The Psychiatric Nurse Practitioner participates in shared decision making with the client and a multi-disciplinary team. They provide services to and make in person visits with clients and their families/custodians/caregivers in their homes and communities and collaborate closely with other service providers and systems with which the client interacts. The role will require some evening availability and rotating on-call coverage. The Adult ACT Team Psychiatric Nurse Practitioner, in conjunction with the Director/Team leader, has clinical responsibility for monitoring recipient treatment and staff delivery of clinical services.
