Qualifications

Job description

Sun River Health provides the highest quality of comprehensive primary, preventative and behavioral health services to all who seek it, regardless of insurance status and ability to pay, especially for the underserved and vulnerable. Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a part-time Psychiatric Nurse Practitioner to join our team supporting the Peekskill and Beacon sites.

A Psychiatric Nurse Practitioner provides psychiatric clinical services for patients and performs job duties in compliance with systems and job specific standards.

Essential Duties and Responsibilities:

  • Consult with MD/Psychiatrist/LCSW on patients via eCW, telephone encounter, and/or with patient visits.
  • Consult with MD/Psychiatrist on medication management, via eCW, telephone encounter, and/or visit with patient as needed to address BH concerns.
  • Provide clinical care (medication management/psych evaluations) for patients to support efforts to achieve care plan goals, in support of ongoing care by MD/Psychiatrist/LCSW, with referral back to ongoing care with clinical team.
  • Participate in case conferencing and high-risk meetings with LCSW/MD/Psychiatrist and care teams.
  • Approval of treatment plans for patients in BH care at Sun River.
  • Participate in efforts to support Sun River’s primary care and BH integration efforts.
  • Ensure appropriate transitions of care with external providers, PCP, hospitals, etc.
  • Respond to Sun River clinical teams for crisis intervention.
  • Interview, diagnose, and evaluate patients referred for psychiatric assessment.
  • Participate as a team member in conferences concerned with diagnosis and treatment planning.
  • Write reports and documentation as required.
  • Consult with other mental health and medical provider staff regarding treatment of patients.
  • Prescribe psychotropic medication as needed and monitor its effectiveness.
  • Provide in-service training to professional and paraprofessional staff.
  • Review case records to monitor quality of care.
  • Demonstrate knowledge and skill necessary to provide care appropriate to the age of the patients served.
  • Demonstrate knowledge of the principles of growth and development over the life span.
  • Identify and categorize each patient’s age-specific grouping of needs, such as those for infant, adolescent, or geriatric patients.
  • Familiarity with the process of diagnosing and treating patients with substance use disorder and make appropriate referrals when necessary.

Minimum Education Requirement: Masters Degree

Minimum Licensure: Nurse Practitioner in Psychiatry and CPR/BLS

Required Skills & Knowledge:
- Knowledge of diagnostic and treatment procedures that nurse practitioners are trained to do and are necessary for practicing in health care centers.
- Competence in delivery of health care to patients (including preventative health education).
- Ability to collaborate with other members of the Health Center in order to maintain high quality standards and achieve client satisfaction.
- Bilingual in both English and Spanish (orally and written) is preferred.

Job type: part-time
Hourly salary: $81.91 - $85.87/ hour
