Job summary

TCA Health in Chicago, IL is seeking a Psychiatric Nurse Practitioner to join our Behavioral Health Department. You will provide direct patient care and support primary care providers in an integrated care model, serving children, adolescents and adults in a fast-paced community health setting.

Ideal candidates hold a Master's in Psychiatric Mental Health Nursing with 3+ years of clinical experience, strong diagnostic skills, and excellent interpersonal abilities. CPR certification is required.

Responsibilities

  • Perform initial and ongoing clinical evaluations
  • Responsible for medication management and treatment planning
  • Perform crisis intervention by telephone for patients during non-office hours as assigned
  • Consult and advise the clinical staff and Clinical Director
  • Manage care coordination with all other appropriate care providers
  • Enter data directly into the Center's electronic medical records system
  • Work with children/adolescents and adults
  • Perform and/or assist in the administration of routine diagnostic/laboratory studies, clinical and therapeutic procedures
  • Develop presumptive nursing diagnoses and implement treatment plans based on advanced nursing assessment/problem identification consistent with established practice guidelines
  • Document assessments of patients' health and illness status
  • Evaluate patient responses to treatment plans and redirect patient care based on findings
  • Assess patient clinical/health education needs and instruct/counsel patients in one-on-one and small group settings regarding health matters
  • Refer patients to clinical/health education classes, as appropriate for more in-depth services
  • Provide telephone care to assigned patients, as necessary
  • Obtain assigned patient vital signs and provide documentation
  • Provide onsite patient care and outreach for our School Based Health Clinic and/or Mobile Health Unit
  • Act as a resource person to other staff members

Position: Psychiatric Nurse Practitioner – Location: Chicago, IL – Department: Behavioral Health – Reports To: Chief Medical Officer

POSITION SUMMARY

The Psychiatric Nurse Practitioner position will play a key role in the fast‑growing Behavioral Health Department of our multi‑discipline primary care community health center. This position will be part of our evolving health services as we move to an integrated care model. In addition to providing direct care to patients, this role will help support the primary care providers and care managers in treating patients with behavioral health problems. The right candidate will thrive in a fast‑paced environment where the first concern is providing outstanding patient care.

ESSENTIAL DUTIES AND RESPONSIBILITIES

QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

EDUCATION AND EXPERIENCE
  • Completion of an accredited Master's Degree program for Psychiatric Nurse Practitioners
  • Experience based on current needs of the practice
CERTIFICATES AND LICENSES
  • CPR Certification

Ability to read, comprehend and communicate detailed instructions. Ability to prepare correspondence and memos. Ability to effectively present information in one‑on‑one and small group situations to patients, physicians and other employees of the organization.
