Job summary

Harbor, Inc. in Perrysburg, OH is seeking a full-time Psychiatric LPN or RN to join our care team. You will assess, diagnose and treat clients receiving medical services, collaborating with a multidisciplinary team.

The role requires an Ohio nursing license (LPN or RN), an Associate's degree, and at least 2 years in psychiatric nursing. CPR/BLS and CPI/NCI certifications within 90 days are required; a valid driver's license is also needed.

Qualifications

• Must have an Ohio LPN or RN license with an Associate's degree in Nursing.
• Two years of psychiatric nursing experience preferred.
• CPR/BLS and CPI/NCI certifications required within 90 days.
• Must have a valid driver's license and proof of auto insurance.

Responsibilities

• Review and update treatment plans for clients receiving pharmacologic management.
• Order and monitor Central Pharmacy medications.
• Coordinate insurance forms and pre-certifications as needed.
• Maintain records regarding medication use and disposal of expired medications.
• Provide medication management and collaborate with the clinical team.
• Assist with crisis intervention and pre-hospital screening as required.
• Complete documentation timely and ensure Harbor forms are used.

Job description

Harbor is looking for a full-time Psychiatric LPN or RN to join our Perrysburg team! This position works with other members of the team in the assessment, diagnosis and treatment of clients receiving medical services.

Position is full-time and 1st shift.

Education/Experience/Other Requirements :

• AnAssociate’sdegree from an accredited school ofNursing with a current LPN or RN license inOhio.
• Two years’ experience working in psychiatric nursing preferred.
• Must be proficient andaccuratein computer use, including Microsoft Word.
• CPR/BLSand NCI/CPI Certificationsrequiredwithin90 daysof employment.
• Must have a valid driver’s license, acceptable driving record, and show proof of personal auto insurance policy to bedeemedinsurable with Harbor’s vehicle insurance carrier.

Essential Job Competencies/Primary Duties:

• Nurses may work at all Harbor sites but will have asite of primary responsibility.Responsibility for a site includes the following:
• reviewsand updatestreatment plans for clients receivingpharmacologic management services ;
• ordersandmonitorsCentral Pharmacy medications;
• coordinating insurance forms and pre-certifications as necessary;
• completes quarterly checks of first aid boxes, bloodborne pathogen kits and maintenance of medical equipment; amount of time necessary to manage a site will bedeterminedby supervisor and the nurse managing the site.
• Provides medication management of clients through:
• dispensing medication samples or Central Pharmacy medications;
• collaborating with other medical and clinical team members on an ongoing basis.
• Responsible for managing medication rooms,maintainingrecords of medicationuseand disposing of expired medications.
• When necessary,assistsother clinical staff with crisis intervention and pre-hospital screening.
• Provides referral/linkage of client toappropriate resources.
• Completes the required number of peer review forms.
• Completes all necessary documentationin a timely manner.
• Typesclinical documentation in the formatappropriateand acceptable to Harbor.
• Provides support for providers byinitiatinglab requestsandcoordinatingcompletion of patient medical documentation requests.
• In providing billable services, fulfills or exceeds unit expectationsestablishedby Harbor.
• Attends meetings asrequiredand serves on committees as requested.
• Provides back upatother sites as needed and requested bysupervisor.
• Maintains certificationto docrisis intervention and pre-hospitalscreening.?
• Participates in professional meetings, workshops,seminarsand other continuing education programs to keep current with professional practice, to contribute to his/her professional growth and tomaintainlicensure.
• May float to other locations to accommodate client or agency needs.
