Job summary

HealthLinc in Elkhart, IN seeks a Psychiatric Nurse Practitioner to provide psychiatric medication management and addiction treatment within an integrated care team. This role includes e-consults to physicians and close collaboration with clinical staff to promote mental wellness and preventive care.

The PMHNP will prescribe medications as appropriate, participate in quality initiatives, and engage with community education efforts while maintaining thorough patient documentation and compliance

Qualifications

• Graduate of an accredited school of nursing and nurse practitioner program.
• Master's degree in Nursing.
• Current Indiana registered nurse license.
• Psychiatric Mental Health Nurse Practitioner (PMHNP).
• Prescriptive authority, DEA certification
• Current CPR certification

Responsibilities

• Utilizes appropriate and effective patient care plans.
• Manages patients to assure continuity of care.
• Provides whole person care, including referrals to other health care providers.
• Provides psychiatric consults and e-consults to the medical and behavioral health providers at HealthLinc.
• Maintains adequate, concise, and legible documentation of patient care.
• Provides appropriate patient education.
• Prescribes Psychiatric Medication and Medication for Addiction Treatment (MAT) when clinically appropriate.
• Participates in QA Committee and attends Team/Clinical meetings.
• Teaches students as assigned and presents in the community when requested.
