Job summary

Continuum of Care in Connecticut seeks an Advanced Practice Registered Nurse (APRN) prescriber to provide outpatient psychiatric services at the CIBH and REST Programs. The APRN conducts evaluations, orders medications, and develops treatment plans within a multidisciplinary team.

Responsibilities include pharmacologic management, lab monitoring, health education, and crisis intervention. Strong reference to HIPAA and documentation is expected, with collaboration across agency and community

Qualifications

• PMHNP (APRN) or equivalent with board certification in psychiatric specialty.
• License to practice in Connecticut in good standing.
• DEA registration including waiver to prescribe buprenorphine.
• Prior experience working with people with severe and persistent mental illness, intellectual disabilities, and/or addiction disorders.
• Valid Connecticut driver’s license with a clean record, maintained throughout employment.
• First Aid/CPR certification or willingness to obtain certification within 90 days of hire, with ongoing maintenance.
• Proficient computer skills, including Microsoft Office and electronic record‑keeping.

Responsibilities

• Performs psychiatric diagnostic evaluations to determine diagnoses, treatment strategies, and care coordination needs.
• Performs pharmacologic treatment strategies, orders medications, monitors, and evaluates progress on psychiatric and/or other medications.
• Assesses need for and orders, monitors, evaluates, and provides clinical interpretation for ongoing evaluation needs for prescribed treatments, including lab tests and specialty consults.
• Completes medication/health education covering informed consent, benefits, risks, side effects, alternative treatments, health/wellness management, and metabolic risk reduction strategies.
• Evaluates clients by conducting intakes, assessing medical histories, and conducting psychological testing; analyzes data and results for diagnosis.
• Prescribes and administers treatments and medications based on client diagnosis.
• Develops treatment plans, evaluates outcomes, and makes necessary adjustments.
• Manages medication refill requests from clients and the agency electronic medication program.
• Performs timely, concurrent, accurate clinical/billing documentation complying with professional standards and state/federal regulations.
• Assures timely access to care, demonstrates scheduling flexibility to meet client needs, and meets applicable productivity standards.
• Provides clinical guidance and crisis intervention support, ensuring comprehensive treatment planning and active supports for clients.
• Coordinates on‑call coverage within the program.
