Job summary

ViaQuest is seeking a Psychiatric Nurse Practitioner to provide long-term care psychiatry for adults 18+ in LTC facilities around Lima, OH. You will conduct face-to-face assessments and ongoing visits, coordinate with facility care teams, and support caregivers to ensure high-quality mental health care.

Requirements include graduation from an accredited NP program and a PMHNP license; geriatric and behavioral health experience is preferred. Outpatient and telepsychiatry experience a plus.

Qualifications

• Graduate of an accredited Nurse Practitioner program.
• PMHNP license required.
• Experience in psychiatric, mental, and behavioral health services.
• Long-term care and/or geriatric experience strongly preferred.
• Outpatient and tele-psychiatry experience a plus.
• Willingness to travel within assigned service area.

Responsibilities

• Performing face-to-face assessments and ongoing psychiatric visits with adult clients in long-term care facilities.
• Maintaining current patient records to clearly document problems, treatment plans, actions, goals, and changes as they occur.
• Educating and supporting caregivers and facility staff.
• Managing and adjusting psychiatric medications to promote stability and well-being.
