This is not your typical psychiatry role.
AbsoluteCare seeks a dynamic, mission-driven Psychiatric Provider to implement team-based collaborative care and population health principles for our medically complex and socially vulnerable members. Our innovative, value-based care model addresses the needs of traditionally underserved patients, reducing unnecessary hospital visits and improving quality of life through an integrated team approach, including primary care providers, care managers, nurses, medical assistants, behavioral health clinicians, and community health workers. Our facilities include an onsite infusion center, pharmacy, and labs, and we actively address social determinants of health like housing, food security, and social isolation.
The Team Psychiatric Provider acts as a psychiatric SME within this integrated team, collaborating to enhance psychiatric care. Responsibilities include supporting patient management via a modified Collaborative Care Model, contributing to behavioral health outcomes through case reviews and team huddles, developing educational materials, and providing direct patient care to our most vulnerable members.
