This is not your typical psychiatry role.

AbsoluteCare seeks a dynamic, mission-driven Psychiatric Provider to implement team-based collaborative care and population health principles for our medically complex and socially vulnerable members. Our innovative, value-based care model addresses the needs of traditionally underserved patients, reducing unnecessary hospital visits and improving quality of life through an integrated team approach, including primary care providers, care managers, nurses, medical assistants, behavioral health clinicians, and community health workers. Our facilities include an onsite infusion center, pharmacy, and labs, and we actively address social determinants of health like housing, food security, and social isolation.

The Team Psychiatric Provider acts as a psychiatric SME within this integrated team, collaborating to enhance psychiatric care. Responsibilities include supporting patient management via a modified Collaborative Care Model, contributing to behavioral health outcomes through case reviews and team huddles, developing educational materials, and providing direct patient care to our most vulnerable members.

Roles & Responsibilities

Provide Clinical Guidance and/or Direct Care for Members with Behavioral Health Needs:

• Guide behavioral health treatment for a specified cohort, collaborating with other clinicians and primary care providers within a modified Collaborative Care Model.
• Collaborate with team members to manage high-risk, high-utilization patients with behavioral health concerns, including screening, therapy, communication strategies, and medication management.
• Educate and support offsite and onsite providers on medical management of behavioral health populations.
• Conduct evaluations, develop treatment plans, and provide diagnoses and recommendations for members requiring psychiatric care.
• Deliver telepsychiatry services to appropriate members.
• Offer peer-to-peer consultations to support member care management.
• Assist with transitional care, emergency department diversion, and crisis intervention.
• Coordinate with inpatient and outpatient mental health providers to ensure seamless communication and care.
• Conduct post-discharge visits for psychiatric inpatients.
• Support medication management, including long-acting injectables (LAIs), and medication reconciliation post-ED or hospitalization.
• Support addiction treatment, emphasizing medication-assisted treatment (MAT) with medications like buprenorphine and naltrexone.
• Participate in developing clinical protocols for MAT and provide direct care for complex patients with substance use disorders.
• Identify and facilitate transitions for members requiring higher levels of care for substance use disorders.
