Telos Health Systems, Inc. in Norfolk, VA is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to lead geriatric care in Skilled Nursing/Assisted Living settings. Choose from part-time or full-time schedules (2–5 days) and enjoy a $5,000 sign-on bonus.

The position offers a salary or wRVU model with a typical full-time range of $140,000–$160,000 per year at 75 visits per week, supported by a strong clinical leadership team and comprehensive benefits.

Qualifications

• Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
• Must have or willing to obtain an active license and DEA #.
• Previous experience in nursing or other medical fields.
• Minimum one-year experience in a Skilled Nursing setting (preferred, not required).
• Ability to build rapport with patients and loved ones.
• Ability to thrive in a fast-paced environment.
• Excellent written and verbal communication skills.
• Strong leadership experience with emphasis on teamwork in a healthcare setting.

Responsabilités

• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.
• Prescribe all necessary medications and treatments.
• Perform comprehensive physical examinations of patients.
• Develop and implement patient management policies and procedures.
• Facilitate referrals to other healthcare professionals and medical facilities.
• Communicate with attending physicians and/or specialists regarding patient care.
• Be comfortable and proficient with documentation and CPT coding.
• Monthly stability visits for long term residents
• Evaluations for patients exhibiting changes in condition.

You will work with the entire facility’s patient population – short and long-term residents – overseeing their day-to-day care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians.
