Job summary

Deer Oaks Behavioral Health — Long-Term Care Clinical Services seeks a Psychiatric Mental Health Nurse Practitioner to conduct in-person psychiatric evaluations for residents in local LTC facilities. You will develop and adjust treatment plans, prescribe psychotropic medications, and implement Medicare-compliant GDR strategies.

You will collaborate with interdisciplinary teams, document findings in the EMR, and contribute to ongoing clinician training and case consultations.

Qualifications

  • Active, unrestricted PMHNP or PMHNP-BC license.
  • Valid DEA registration.
  • Experience in LTC settings preferred.
  • Commitment to timely documentation and high-quality psychiatric care.

Responsibilities

  • Conduct comprehensive psychiatric evaluations and mental health assessments for LTC residents.
  • Develop, implement, and adjust individualized treatment plans.
  • Prescribe and manage psychotropic medications per Medicare and LTC guidelines.
  • Implement Medicare-compliant gradual dose reduction (GDR) strategies.
  • Monitor and treat chronic mental health conditions (depression, anxiety, bipolar, schizophrenia, dementia behaviors).
  • Collaborate with nursing, medical, social services, rehabilitation and facility leadership.
  • Document assessments, plans, and progress notes in the EMR.

Psychiatric Mental Health Nurse Practitioner (PMHNP)

In this role, you will be assigned to a facility or facilities in your local community, allowing you to build meaningful connections with the individuals you serve. You will have the opportunity to conduct in-person psychiatric assessments, medication management, and implementing Medicare compliant gradual dose reduction strategies for psychotropic medications. Clinical Care
  • Conduct comprehensive psychiatric evaluations and mental health assessments for LTC residents
  • Develop, implement, and adjust individualized treatment plans
  • Prescribe and manage psychotropic medications in accordance with Medicare and LTC best practices
  • Implement and document Medicare-compliant gradual dose reduction (GDR) strategies
  • Monitor and treat chronic mental health conditions including depression, anxiety, bipolar disorder, schizophrenia, and dementia-related behavioral disturbances
  • Collaborate with interdisciplinary teams—nursing, medical providers, social services, rehabilitation, and facility leadership
  • Complete accurate documentation of assessments, treatment plans, and progress notes using the electronic medical record (EMR) system
Interdisciplinary Collaboration
  • Work closely with nursing, social services, rehabilitation, and medical providers to ensure coordinated psychiatric care.
  • Participate in care plan discussions, facility meetings, and case collaborations as needed.
Documentation & Compliance
  • Maintain complete and timely documentation using electronic medical records (EMR).
  • Follow ethical, professional, and regulatory guidelines for psychiatric documentation and care delivery.
Facility Partnership
  • Build strong, professional relationships with facility staff and leadership.
  • Serve as a psychiatric resource and trusted partner for supporting residents’ mental and emotional needs.
