Job summary

Valley Health Partners in Allentown, PA is seeking a Psychiatric Case Worker to provide case management, counseling support, patient and family education, and crisis intervention under supervision.

The role includes utilization review, discharge planning, and documentation in accordance with program and regulatory standards; CPR/BLS training is required on hire.

Qualifications

• Bachelor’s Degree in Social Work, Psychology, Counseling, or related mental health field; Master’s Degree preferred.

• Provides case management services, including linkages among departmental and community resources, under the supervision of practice leadership.
• Provides supportive counseling, acts as patient/family education and liaison, and assists with crisis intervention under the supervision of practice leadership.
• Provides utilization review and discharge planning services using a comprehensive grasp of VHP community supports.
• Assists with information gathering to support patient assessment, initial/ongoing treatment plan, or for ongoing treatment issues under.
• Documents activities in a timely fashion and in adherence with program, departmental, and regulatory standards.
• Takes on other duties as assigned

Education:

• 2 years of experience in a mental health setting.
