This position must be able to demonstrate the knowledge and skills necessary to support/provide care appropriate to the age of the patient/resident served on unit assigned. The individual must have knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's/residents status and interpret the appropriate information needed to identify each patient's/resident's requirements relative to age-specific needs and to provide the care needed as described in the departmental policy and procedure guide.What you will dAssesses and treats children, adolescents, and adults seeking psychiatric and/or chemical dependency treatment. Communicates patient, resident, family needs to physicians accurately and completely. Familiarizes self with the plan of care and provides relevant insight into overall success or areas of improvement.Collaborate with referral sources, patients, families, and team members regarding care coordination. Provides follow-up actions as needed.Serves as the initial contact for patients, families, referral sources and/or allied agencies seeking intervention or support. Provides complete and accurate Intake Screening Reports of the patient and/or caller complaint and condition, including initial observation made upon arrival, reflective of the patient’s status and age, as described in the policy and procedure guide.
