Job summary

Care For the Homeless (CFH) in New York City seeks a Nurse Practitioner to join the PCMH team and deliver integrated primary care to people experiencing homelessness. You will collaborate with physicians, case managers, and health educators to coordinate medical care across clinics and shelters.

The role emphasizes compassionate, nontraditional setting care, engaging clients with behavioral health needs and providing provider-level leadership, training, and outreach to improve health outcomes

Qualifications

• Must hold New York State Nurse Practitioner license.
• DEA registration required.
• Flexible and able to function in nontraditional settings.

Responsibilities

• Participates as a member of committees, including peer review, as requested by the CMO
• Chart completion is consistent with CFH P/P
• Reaches out and engages clients with appropriate concern for their behavioral health care needs
• Coordinates medical care with PCP for chronic and acute non-behavioral health conditions
• Advises and counsels clients about their physical and mental health; coordinates care with case management and health education as clinically indicated
• Provides consultation and training to staff and volunteers where appropriate.
• Assists with the identification, counseling, and care coordination, with case management and health education, of mental health and substance abuse problems.
• Works in a non-traditional environment; e.g., a non-clinic facility where homeless people congregate.

Care For the Homeless (CFH) is a nationally recognized nonprofit leading the way in caring for those experiencing homelessness and housing instability in New York City. Since 1985, as the city’s first “Health Care for the Homeless” program funded by the Robert Wood Johnson Foundation and Pew Charitable Trusts, we have integrated healthcare, shelter, and social services in a coordinated, innovative model of care that meets people where they are. By providing comprehensive, compassionate support across 16 community health centers and five shelters throughout the city, we improve health outcomes and create pathways to stability. SUMMARY

Reports to the collaborating physician at the contracted facility.

This employee will work as part of the PCMH team with the following essential duties and responsibilities
