Job summary

Care For the Homeless (CFH) seeks a Nurse Practitioner to join its PCMH team in New York City. The role involves collaborating with a physician, conducting histories, exams, and HIV testing on-site, and delivering integrated health care in non-traditional settings.

You will engage clients, assess illnesses, provide counseling, coordinate care, and contribute to staff training while serving unhoused populations across CFH's centers and shelters.

Responsibilities

• Participates as a member of CFH committees, including peer review.
• Chart completion is consistent with CFH P/P.
• Reaches out and engages clients with appropriate concern for their health and other needs.
• Obtains health histories and performs physical examinations and screenings, including testing for HIV on-site.
• Assesses and treats common, acute, and chronic illnesses, including HIV/AIDS; refer clients when necessary to the provider's facility or another appropriate facility; follow up on such referrals.
• Conducts health screening services.
• Makes every effort to provide and assure ongoing medical service, including a continuum of care for persons with HIV/AIDS
• Advises and counsels clients about their physical and mental health.
• Provides consultation and training to on-site staff and volunteers where appropriate.
• Assists with the identification, counseling, and care coordination, with case management and health education, of mental health and substance abuse problems.
• Works in a non-traditional environment; e.g., the non-clinic facility where homeless people congregate

Since 1985, as the city7s first “Health Care for the Homeless” program funded by the Robert Wood Johnson Foundation and Pew Charitable Trusts, we have integrated healthcare, shelter, and social services in a coordinated, innovative model of care that meets people where they are. SUMMARY: Reports to the collaborating physician at the contracted facility.

This employee will work as part of the PCMH team with the following essential duties and responsibilities:
