Job summary

Meridian Associates, LLC is seeking a dedicated PSR professional to provide goal-oriented assessment, planning, and maintenance of daily living skills. You will coordinate with therapists, families, and guardians to implement Recovery Treatment Plans and support successful community integration.

The role requires strong communication, ethics, and the ability to work in diverse settings including clients’ homes and clinical environments.

Qualifications

• Bachelor’s degree or higher in counseling, social work, psychology, nursing, rehabilitation, or related human services field.
• High School Diploma or GED with one of: Certified Psychiatric Rehabilitation Practitioner, Certified Recovery Support Specialist, or Certified Behavioral Health Technician.

Responsibilities

• Performs assessment with client, facility staff, and guardians when applicable.
• Develops goal-oriented PSR Recovery Treatment Plan and activities to support client needs.
• Works with clients in their home environment when appropriate.
• Participates in staffing to tailor services to client strengths and needs.
• Communicates with client’s therapist and family members.
• Monitors progress through regular discussions with client, therapist, and family.
• Adheres to healthcare ethics and PHI/Billing policies.
• Participates in multidisciplinary trainings and meetings.
