Responsibilities

• Site and Provider Credentialing: Maintains data for all providers.
• Completes credentialing and re-credentialing applications and follows up as needed.
• Maintains copies of licenses, DEA certificates, malpractice coverage and other documents.
• Keeps knowledge of health plan requirements for credentialing providers.
• Sets up and maintains provider information in credentialing databases.
• Tracks license and certification expirations for all providers.
• Ensures practice addresses are current with health plans.
• Maintains collaborative agreements with providers.
