Our outpatient services specialize in treatment for mental health and substance use disorders, offering treatment and skills groups, as well as community resources.

Outpatient programs provide trauma-informed care aimed at reducing the harmful impacts of substance use. Participants and their counselors work collaboratively to design an integrated, individualized, strengths-based treatment experience tailored to each client’s needs and goals. Services include individual and group therapy, case management, treatment planning, drug and alcohol counseling, crisis intervention, and connection to social support services.

Job Summary:

Prototypes’ outpatient mental health services provide comprehensive mental health care for the community, from prevention and early intervention to intensive outpatient services, including Full-Service Partnership (FSP). Services are delivered at our sites and in the community, encompassing individual and group services, medication support, case management, and coordination with other healthcare providers.

The Case Manager coordinates care across HealthRIGHT 360’s continuum, collaborating with human services providers and clients to complete intake documents, outcome measures, and benefit applications. The role involves working with a multidisciplinary team to ensure optimal treatment outcomes, providing services in outpatient offices, residential treatment, or in the field based on client and program needs.

The Case Manager maintains a caseload and links clients to services addressing benefits, mental and physical health, employment, probation, DCFS, housing, community resources, substance use disorder services, and aftercare. They support clients in increasing self-efficacy, self-advocacy, life skills, coping strategies, and community reintegration, working collaboratively with the team to ensure effective treatment.

Key Responsibilities:

  • Conduct intake services, including opening treatment episodes, communicating with EHR and Billing teams, and collaborating with supervisors.
  • Assist clients with Medi-Cal applications or transfers.
  • Gather healthcare provider information and manage releases of information.
  • Coordinate communication and linkage with external services.
  • Serve as contact for medication-only or Wellness services, maintaining client records.
  • Collaborate on outcome measures like WHO-QOL, OMAs, KECs.
  • Complete DMH NET assessments for clinical needs.
  • Deliver services in individual, family, and group formats.
  • Seek clinical consultation as needed, especially in high-risk situations.
  • Conduct and document UA tests when assigned.
  • Maintain communication with the treatment team through meetings and conferences.
  • Provide services in outpatient, residential, or field settings, including outside business hours.
  • Meet client care hours (minimum 25 hours/week).
  • Provide translation services as needed.
  • Support parents and caregivers in navigating health systems and benefits.
  • Offer field-based services as required.
  • Assist with outreach, engagement, and client intakes, including in the field.
  • Complete and submit program documentation, invoices, and reports timely and in compliance with policies and standards.

Qualifications:

Required:

  • High school diploma or equivalent.
  • At least one year of experience in human services or related expertise in mental health, substance use, family treatment, or recovery.
  • Knowledge of trauma-informed care.
  • Valid driver’s license and proof of insurance.
  • Two years’ experience in human services or related fields, with expertise in substance abuse treatment and recovery.
  • No active parole or probation.
  • Successful background check and live scan.
  • Full COVID-19 vaccination, including booster shots (medical/religious exemptions available).
