Job summary

Via Care Community Health Center seeks a Promotora to serve as a trusted community advocate, connecting individuals and families to healthcare services and resources. You will support enrollment, provide health education, and promote Via Care's programs through outreach and partnerships.

Responsibilities include conducting culturally responsive education, building relationships with schools, faith-based groups, and local organizations, recruiting participants, and assisting with appointment

Qualifications

• High school diploma or GED.
• Bilingual in English and Spanish.
• 1 year of experience in community outreach, health education, enrollment services, customer service, public health, or related field (volunteer experience may be considered).
• Strong communication and interpersonal skills.
• Ability to build trust and establish relationships with diverse community members.
• Ability to travel throughout Via Care's service areas and attend occasional evening or weekend events.
• Reliability automobile preferred.
• TB clearance and vaccinations as required.

Responsibilities

• Conduct community outreach activities to promote Via Care's healthcare services and programs.
• Build and maintain positive relationships with community members, schools, local businesses, faith-based organizations, housing sites, and community-based organizations.
• Recruit participants for health education activities, enrollment events, and community programs.
• Represent Via Care at health fairs, community events, educational workshops, and outreach activities.
• Provide culturally responsive health education and information regarding available healthcare services, including primary care, dental, behavioral health, women's health, senior services, and urgent care.
• Assist community members in accessing healthcare services, community resources, and preventive care programs.
• Support patient enrollment and registration efforts, including appointment scheduling and referrals as needed.
• Conduct outreach calls and follow-up activities to encourage participation in services and community programs.
• Distribute educational materials and promote awareness of Via Care programs and services.
• Maintain accurate records of outreach activities, contacts, referrals, and event participation.
• Collaborate with Outreach and Enrollment staff to identify opportunities for community engagement and growth.
• Participate in required trainings, staff meetings, and outreach events.
• Maintain confidentiality and comply with HIPAA requirements.
• Perform other duties as assigned.
