Qualifications

  • High school diploma or GED required for Case Manager I.
  • Bachelor's degree in counseling, social work or related field preferred; will accept equivalent experience.
  • Case Manager II requires Bachelor’s degree in counseling or social work; will accept equivalent experience.
  • Knowledge of community resources preferred.
  • Cultural sensitivity, awareness and knowledge.
  • Strong Communication Skills
  • Must be 21 years old or over.
  • Bilingual Spanish required
  • A valid California driver’s license is required of staff who have regular driving duties, and the driving record must meet agency requirements.

Responsibilities

  • Identify high-risk needs in youth and provide resources and referral information.
  • Provide access, advocacy, resources and linkage to clients.
  • Provide initial screenings and treatment planning for clients.
  • Attend community events and provide outreach services; evenings and weekends may be required.
  • Maintain records and tracking systems meeting agency, Joint Commission and contract standards.
  • Deliver education on mental health resources and healthy lifestyles in the community.
  • Administer program measurements tools and surveys to track and report program data and outcomes.

Qualifications

  • Case Manager I - High School Diploma or GED required.

Essential Job Functions

  • Operate an agency or personal vehicle for work-related activities which may include, client visits, meetings, trainings, or other agency-related appointments
  • Ensure compliance with driving requirements, including maintaining a valid driver’s license, current vehicle registration, and proof of insurance as required by agency policy.
  • Ability to communicate ideas in oral and written form.
  • Protect and honor individual integrity through confidentiality.
  • Ability to work in a harmonious manner with subordinates, supervisors, and others.
  • Provide parent and staff training.
  • Provide classroom observations.
  • Provide staff consultation and training.
  • Attend Child Success Team meetings and program meetings as needed.
  • Physical, mental and emotional health adequate to perform duties.

Responsibilities

  • Must implement program goals and objectives to meet and maintain contractual requirements and productivity.
  • Interact productively within a multi-disciplinary team and other community organizations.
  • Maintain record and tracking systems that meet standards of the agency, The Joint Commission and contract.
  • Teach and model appropriate intervention skills for family.
  • Delivers education on mental health resources and healthy lifestyles in the community.
  • Administer program measurements tools and surveys to track and report program data and outcomes.
  • Responsible for preventing and controlling infection.
  • Responsible for maintaining a culture of quality and safety.
  • Effectively use de-escalation skills and techniques for crisis prevention
  • Properly administer the C-SSRS to help prevent suicide.

Other

  • Attend all required meetings and trainings.
  • Report any suspected child or dependent adult/elder abuse or neglect immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.
  • Report any client imminent danger to self or to others or gravely disabled immediately to direct supervisor or utilize the chain of command if supervisor is unavailable.
  • Other duties as assigned.
