Job summary

Trinity Health's St. Joseph's Health in Syracuse is seeking Registered Nurses for the Progressive Care Unit 3-4.

This full-time role offers rotating shifts, a supportive team, and professional growth within a respected medical network. You will assess and coordinate care from admission to discharge, maintain documentation, and participate in ongoing education and specialty training such as CORE EKG, ACLS, ventilator care.

Qualifications

• Graduation from an accredited school of nursing and current licensure in the State of New York with at least 1 year of experience.
• BSN degree preferred, according to NYS requirements.

Responsibilities

• Collaborate, communicate and facilitate optimal provision of care with assistance of computerized/digital platforms.
• Assess, plan, implement, coordinate, monitor and evaluate the patient’s plan of care from admission to discharge.
• Utilize clinical knowledge and critical thinking to coordinate and support the care plan.
• Ensure seamless, effective transition of care across the continuum.
• Monitor quality metrics and maintain complete, accurate documentation in the patient record.
• Act as mentor, clinician, educator, consultant/collaborator and navigator.
