Job summary

Salt Lake Behavioral Health is hiring a Full-Time Program Therapist (Social Worker/ACMHC) for our Rehab units. You will perform bio-psychosocial assessments, develop treatment plans, provide individual and group therapy, and assist in discharge planning.

Collaboration with the treatment team, families, and community resources is essential. The role requires a Master's in Social Work or related field, Utah licensure (CSW/LCSW/ACMHC/CMHC/AMFT/LMFT) and EMDR training, plus CPR certification.

Qualifications

• Master's degree in Social Work or related clinical field required.
• Licensed as CSW/LCSW/ACMHC/CMHC/AMFT/LMFT in Utah and EMDR trained.
• CPR certification required and restraints/seclusions training completed within 90 days.

Responsibilities

• Perform bio-psychosocial assessments for new admissions.
• Develop and implement individualized treatment plans.
• Provide daily individual therapy and psycho-educational groups.
• Discharge planning with patient families and support systems.
• Complete timely documentation to hospital standards.

Job Description:

As part of the social services team, this is a position that will complete bio-psychosocial assessments, develop treatment plans, provide individual/group therapy and assist in discharge planning. This position will work collaboratively with the treatment team, families, community resources, and other healthcare providers.

Job Essentials:

• Prioritize daily tasks including new admissions and discharging patients along with seeing patients on given case load.
• Participate daily in a multi-disciplinary treatment team meeting.
• Conducts psychosocial assessments, ASAM’s and health disparities form for new admissions.
• Educate, communicate and coordinate discharge plans with patient’s families and support systems.
• Provide daily individual therapy along with psycho-educational groups.
• Complete discharge planning including crisis safety plans, family meetings and discharge summaries.
• Complete documentation in a timely manner according to the given standards of the hospital.

Compensation:

• Pay Range CSW/ACMHC (Exempt): $32.30-$42.80/hr
• Pay Range LCSW/CMHC (Exempt): $36.30-$46.80/hr

Qualifications

Education: Master's Degree from an accredited college or university, in Social Work or a clinical related mental health field required.

License: CSW, LCSW, ACMHC, CMHC, AMFT, LMFT with the state of Utah. EMDR trained.

Additional Requirements: CPR certification is required. You will need to successfully complete our current training for restraints and seclusions. This training will need to be done prior to assisting in any patient restraints or seclusions and within 90 days of employment.
