Job summary

Qualifications

  • Six (6) months of experience in the Mental Health or Social Services field preferred.
  • Current CPR and first aid certificate or willingness to be trained required within the first 90 days of employment.
  • Trained in de-escalation training or willing to be trained required within the first 90 days of employment.
  • Ability to be credentialed through Sacramento County.
  • Willingness to transport patients daily.
  • Must have a valid CA driver’s license and an insurable driving record.
  • Must read and interpret a map and/or use GPS navigation safely while driving hands-free.
  • Ability to maintain confidentiality and adhere to professional ethics.
  • Fluency in additional languages preferred.
  • Excellent customer service experience and strong listening skills.
  • Crisis management and critical thinking skills for behavioral health support.
  • Commitment to serving underserved populations.

Responsibilities

  • Provide daily transportation for patients in crisis to CRBH and return to discharge location.
  • Assist patients experiencing homelessness and co-occurring disorders at a high acuity facility.
  • Work with the multi-disciplinary team to complete discharge planning for patients.
  • Document in EHR pickup and discharge location and assessment of safe transport.
  • Serve as a lead team member for de-escalation when required.
  • Assist with the triage process for new intakes.
  • Ensure patient belongings are labeled and stored according to procedures.
  • Interact with patients and partners courteously and respectfully.
  • Ensure the van is maintained, disinfected after use, and fuel is adequate.
  • Transport 12-20 patients per shift for intakes and discharges.

Position Summary

The Program Support Specialist supports the Crisis Receiving for Behavioral Health (CRBH) program by providing triage, transportation, de-escalation, and discharge planning to patients having a behavioral health crisis in Sacramento County.

Required Qualifications
  • High School Diploma or equivalent required.
  • Six (6) months of experience in the Mental Health or Social Services field preferred.

  • Excellent customer service experience, including active listening, problem-solving, and calmness in emotional or stressful situations.
  • Critical thinking and crisis management skills to support patients with mental health and substance issues.
  • Demonstrate commitment to providing services for the underserved and sensitivity, working with people from low-income populations with diverse educational, lifestyle, ethnic, and cultural origins; disabled, homeless, substance users, HIV (AIDS) infected, and/or physiologically impaired.
  • Commitment to HIPAA and ability to abide by standards of professional ethics and maintain confidentiality.
  • Adherence to infection control procedures, including but not limited to standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking, and social distancing.
  • Assist and support the Center/Department/Program to meet standards of High Reliability.
  • Must be willing to successfully complete the credentialing and onboarding process.
Essential Responsibilities
  • Provide daily transportation for patients in crisis to CRBH and return service to their designated discharge location.
  • Assist patients experiencing homelessness and co-occurring disorders such as mental health, chronic medical conditions, and substance use disorders at a high acuity care facility requiring 24/7 patient support and monitoring.
  • Work with the multi-disciplinary team to complete discharge planning for patients.
  • Document in EHR pickup and discharge location and assessment of safe transport.
  • Serve as a lead team member for de-escalation, when required.
  • Assist with the triage process for new intakes.
  • Ensure the patient's belongings are properly labeled according to WSH procedures and stored in provided storage.
  • Interact with patients and partners courteously and respectfully.
  • Ensure the van is properly maintained:
    • Ensure the van is in safe operating condition before each use.
    • Disinfect after each use.
    • Notify the supervisor of necessary repairs or safety issues.
    • Clean interior and exterior of van daily (including windows).
    • Ensure the van has sufficient fuel for the trip.
  • Be able to work in a fast-paced environment. Transport 12-20 patients a shift for intakes and discharges to and from CRBH.
  • Keeping the workload of intake and discharge transports organized.
  • Must always present a professional appearance per WellSpace Health’s personal appearance policy.
  • Keep a record of vehicle mileage and condition.
  • Exercise good judgment and extreme caution when operating a motor vehicle. Provide careful navigation downtown and during commuting hours.
  • Establish and maintain effective working relationships with partnering agencies.
  • Transport to all areas in Sacramento County, including but not limited to hospitals, patient's homes, treatment centers, homeless encampments, and dark areas in the evening or night.
  • Communicate and coordinate with multi-disciplinary team, including but not limited to RN, LVN, SUD Counselor, Clinicians, and Mental Health Resource Specialists for patient coordination and care.
  • Assist with CRBH daily operations such as laundry, food distribution, cleaning, etc.
  • Other duties as assigned.

Physical Demands and Work Environment

The work environment is characteristic of a medical clinic environment. While performing the duties of this job, the employee is frequently required to sit; use hands and arms to reach. The employee is also required to stand; walk; and reach with hands and arms. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus. The employee must also possess hearing and speech to communicate in person and over the phone. The noise level in the work environment is usually quiet.

The employee may be in contact with individuals and families in crisis who may be ill, using substances and/or not attentive to personal health and safety for themselves or their homes. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and the lack of personal care. The employee may also be exposed to bodily fluids (blood, urine) and hazardous chemicals.
