Job summary

WellSpace Health in Sacramento, CA seeks an LVN to join a 24-hour crisis receiving team serving patients with behavioral health crises and co-occurring conditions. The role includes direct patient care, assessments, vital signs, and documentation in the EHR, with on-call, weekend and holiday coverage.

The ideal candidate holds a California LVN license, First Aid/CPR, strong interpersonal and customer service skills, and the ability to work calmly in emergencies.

Qualifications

• Current license as Licensed Vocational Nurse with the State of California required.
• Two plus years of clinical experience or an equivalent education/experience.
• New graduates with licenses encouraged to apply.
• Fluency in additional languages preferred.
• Current First Aid and CPR Certification.
• Knowledge of medications, usages, interactions, and effects of substance use.
• Observe, assess, and record symptoms; react calmly in emergencies.
• Experience educating patients on care and treatment.
• Maintain detailed records.
• Excellent customer service experience with active listening and problem solving.
• PC literate, including electronic health records, word processing, and e-mail.

Responsibilities

• Act as on-site medical professional providing direct patient care according to policies and protocols.
• Perform LVN duties for patients, prepare rooms, and intake assessments including vital signs.
• Engage with community members in behavioral health crisis, homelessness, and substance use.
• Encourage patient well-being through health promotion.
• Follow standard policies for treatments and conditions.
• Keep accurate records for funding and inspections.
• Maintain files for regulatory compliance and inspection.
• Inventory medications and supplies; rotate to minimize expirations.
• Review patient charts and prepare documentation.
• Record all patient interactions in the EHR system for follow-up and billing.
• Assist with daily operations, laundry, food distribution, cleaning, etc.
• Perform other nursing duties for patients under influence of substances or in crisis.

Position Summary

Work as part of a multi-disciplinary team with patients experiencing a behavioral health crisis with co-occurring disorders such as mental health, chronic medical conditions, and substance use disorders in a 24-hour crisis receiving environment offering patient support and monitoring. We welcome newly licensed graduates to apply.

Required Qualifications

• Current license as Licensed Vocational Nurse with the State of California required.
• Two plus years of clinical experience with OR an equivalent combination of education and experience preferred.
• New graduates with licenses are encouraged to apply.
• Fluency in additional languages preferred.
• Current First Aid and CPR Certification.
• Knowledge of community resources, including Supplemental Security Income (SSI), and the county's General Assistance Program.
• Knowledge of medications, usages, interactions, and effects of substance use.
• Ability to observe, assess, and record symptoms, reactions, and progress; react calmly and effectively in emergencies.
• Experience educating patients and providing instructions on proper care and treatment.
• Maintain detailed records.
• Exceptional interpersonal skills.
• Excellent customer service experience, including active listening, problem-solving, and calmness in emotional or stressful situations.
• Computer (PC) literate, including electronic health record, word processing, and e-mail, required.
• Communicate clearly and concisely, both verbally and in writing, and demonstrate a high level of listening skills.
• Adherence to infection control procedures, including but not limited to standard precautions of temperature monitoring, hand washing, symptom self-monitoring, masking and social distancing.
• Assist and support the Center/Department/Program to meet standards of High Reliability.
• Ability to work holidays, weekends, and alternate shifts as needed to maintain required staffing levels.
• Successful completion of all required organizational and Crisis program training.
• Must be willing to successfully complete the credentialing process.

Essential Responsibilities

• Keep accurate records for reporting to various funding and inspecting agencies.
• Maintain files for regulatory compliance and inspection.
• Inventory medications and supplies; rotate inventory regularly to minimize expirations.
• Review patient medical charts and prepare documentation.
• Record all patient interactions in the EHR system according to established timelines and ensure accuracy for patient follow‑up and billing.

The work environment is characteristic of a medical clinic environment.
