Job summary

Mentor Community Services, part of the Sevita family, is hiring a Program Supervisor in Joliet, IL. This full-time role runs Sunday–Thursday 7a-3p and pays $21.45 hourly.

You will supervise Caregivers, support ISP implementation, and assist with meals, medications, and personal care while engaging individuals in community activities. Applicants should have a High School diploma or equivalent, 1 year related experience, and a valid driver’s license with insured vehicle.

Qualifications

• High School diploma or equivalent required.
• One year related work experience.
• Must be 18 years or older.
• Valid driver’s license, vehicle registration, and auto insurance.
• Any licensure/certification required by regulator.

Responsibilities

• Facilitate ISP implementation and help with staff hiring, training, scheduling, and oversight.
• Supervise a team of Caregivers supporting individuals in the program.
• Provide direct support including meal prep, medications administration, and personal care.
• Duties split between direct support, program activities, and supervision.
• Assist with community outings or transportation to work or activities.

Program Supervisor

Full time schedule: Sunday -Thursday 7a-3p

$21.45 hourly

The Program Supervisor roles requires previous Direct Support Professional (DSP) or closely related healthcare experience. Previous management experience preferred.

• Facilitate the implementation of Individual Service Plans (ISP) and participate in a range of administrative duties including staff hiring, training, scheduling, and oversight.
• Supervise a team of Caregivers supporting individuals we serve in the program.
• Provide support to individuals receiving services including: assistance with daily activities such as meal preparation and planning; medication administration; and personal care.
• Duties are split between providing direct support, professional or program activities, and supervision.
• Facilitate community involvement by accompanying individuals on outings in the community or providing transportation to work or other activities.

Qualifications:

• High School diploma or equivalent.
• One year related work experience.
• Must be 18 years or older.
• Current driver's license, car registration, and auto insurance.
• Other licensure or certification where required by regulatory authority.
• Excellent communication skills with an ability to establish rapport with team members and those we serve.
• Strong organizational abilities to ensure staffing and schedules are maintained.
