Job summary

REM Community Services, part of Sevita, seeks a Program Supervisor in Baxter, MN. You will oversee ISP implementation, supervise a Caregiver team, and support individuals in daily activities while coordinating scheduling and trainings.

The role requires a high school diploma, 1 year of related experience, a valid driver’s license, and strong communication and organizational skills. Opportunities include full-time and part-time schedules.

Responsibilities

• Facilitate ISP implementation and assist with staff hiring, training, scheduling, and oversight.

• Provide support with daily activities, meal prep, medication administration, and personal care.
• Balance direct support with program activities and supervision.
• Coordinate community involvement and transportation to work or activities.

PS Baxter (40) Days Monday, Tuesday, Wednesday, Friday 7am - 3pm, Every other Saturday and Sunday, 8am - 4pm
Must have valid Driver's License for at least 1 year post permit and a clean Motor Vehicle Record that meet's Sevita insurability policy
Program Supervisor

• Facilitate the implementation of Individual Service Plans (ISP) and participate in a range of administrative duties including staff hiring, training, scheduling, and oversight.
• Supervise a team of Caregivers supporting individuals we serve in the program.
• Provide support to individuals receiving services including: assistance with daily activities such as meal preparation and planning; medication administration; and personal care.
• Duties are split between providing direct support, professional or program activities, and supervision.
• Facilitate community involvement by accompanying individuals on outings in the community or providing transportation to work or other activities.
  

Qualifications

• High School diploma or equivalent.
• One year related work experience.
• Must be 18 years or older.
• Current driver's license, car registration, and auto insurance.
• Other licensure or certification where required by regulatory authority.
• Excellent communication skills with an ability to establish rapport with team members and those we serve.
• Strong organizational abilities to ensure staffing and schedules are maintained.
