Job summary

Pediatric Specialty Care (PSC) in Point Pleasant, PA, is seeking a Program Specialist to coordinate services for medically fragile children, develop individualized plans, and advocate for optimal outcomes in a home-like setting.

This hands-on role emphasizes interdisciplinary collaboration, family engagement, and ensuring plans meet PA 3800/6400 and DHS standards, with 7.5-hour shift options. Benefits include health, 401K, PTO, and EAP.

Qualifications

• Bachelor’s degree in Psychology, Sociology, Human Services or related field.
• 2 years IDD experience required with a Bachelor’s degree, or 4 years IDD experience with an Associate degree.
• Experience working with individuals with Intellectual and Developmental Disabilities (IDD).

Responsibilities

• Develop, manage, and update ISP/IHP plans and 6-month reviews.
• Coordinate interdisciplinary input for comprehensive, person-centered care planning.
• Lead meetings in compliance with PA 3800/6400 and DHS regulations.
• Train staff on individualized plans, behavioral supports, and safety protocols.
• Monitor monthly and quarterly progress toward outcomes.
• Maintain communication with families, schools, and external providers.

Responsibilities

What You’ll Do - As a Program Specialist, you will serve as a key coordinator and advocate for assigned individuals:

• Develop, manage, and update Individual Support Plans (ISP/IHP) and 6-month reviews

• Coordinate interdisciplinary input to ensure comprehensive, person-centered care planning

• Lead and facilitate required meetings in compliance with PA 3800/6400 and DHS regulations

• Train and support staff on individualized plans, behavioral supports, and safety protocols

• Monitor and document monthly and quarterly progress toward outcomes

• Ensure all required services are delivered and aligned with each child’s goals

• Maintain consistent communication with families, schools, and external providers

• Participate in interdisciplinary, QA, and safety meetings

• Support participation in community-based activities and life experiences

Qualifications

What You Bring as Program Specialist

• Bachelor’s degree in Psychology, Sociology, Human Services, or related field + 2 years IDD experienceOR

• Associate degree (60+ credits) in related field + 4 years IDD experience

• Experience working directly with individuals with Intellectual and Developmental Disabilities (IDD)

• Strong organizational, documentation, and coordination skills

• Excellent communication and relationship-building ability

• Comfort working across multiple disciplines and developmental levels

• Proficiency with Microsoft Office and general computer systems

• Valid driver’s license; ability to attend off-site meetings

• CPR/First Aid Certification (or willingness to obtain)

• Must be 21 years of age or older
