Job description

Description

This position involves providing direct services to individuals. The program is funded to care for patients/clients who do not fit well within the standard medical model. The role requires collaboration with medical providers, primary care teams, and other agencies to improve patient outcomes and care. It aims to reduce health disparities caused by COVID-19 or other health emergencies and to advocate for social and environmental needs. The position works closely with the health center site, PCPs, Local Health Departments, and Community-Based Organizations to provide wrap-around services and build trust with socially vulnerable populations.

Examples of Duties
  1. Support - Direct Patient Interaction 100%
  2. Explain program requirements and guidelines to the general public.
  3. Develop community resource materials reflective of the local community.
  4. Conduct interviews and intake assessments to determine individual needs.
  5. Assist patients in setting and achieving personal goals via various communication methods.
  6. Support skill-building interventions to help patients utilize resources, manage symptoms, and develop independence.
  7. Connect patients to community resources such as housing, medical care, education, and vocational training.
  8. Assist with transportation to appointments.
  9. Maintain written records as required by the program.
  10. Participate in administrative and training meetings to adhere to policies and protocols.
  11. Complete other duties as assigned.
Qualifications

CREDENTIALS TO BE VERIFIED BY PLACEMENT OFFICER

  1. Any combination totaling three years (36 months) from the following categories:
    1. Coursework in social work or behavioral sciences, or a related field, with conversion details provided.
    2. Progressively responsible work experience in social service/counseling at a paraprofessional level under qualified supervision.
Knowledge, Skills and Abilities (KSAs)
  1. Understanding of human nature.
  2. Ability to interview individuals to assess needs.
  3. Skill in organizing and maintaining records.
  4. Ability to obtain necessary services for participants.
  5. Ability to establish effective relationships with participants and staff.
