Qualifications

• Current licensure as R.N. or L.P.N. in Pennsylvania.
• Current valid driver’s license.
• Current CPR certification.
• Strong communication skills including written and verbal English proficiency.

Responsibilities

• Perform assessments of clients’ health status on admission and on an ongoing basis.
• Communicate and coordinate health information with nurses, program staff, and external providers.
• Screen and triage illness/injury and arrange appropriate care.
• Coordinate and monitor medical/dental/psychiatric care and follow-up.
• Train individuals and staff to implement health interventions and coordinate with the program team.
