Role Description

This is a full-time remote role for a Program Manager, Population Health. The Program Manager will oversee the design, implementation, and evaluation of population health initiatives. Responsibilities include collaborating with healthcare providers, community organizations, and stakeholders to improve health outcomes, reduce costs, and enhance care quality. Key duties involve analyzing population health data, developing intervention programs, monitoring performance, and reporting outcomes to leadership.

Qualifications
  • A Bachelor’s or Master’s degree in Public Health, Healthcare Administration, or a related field (required).
  • 3+ years of experience in population health, public health, or healthcare program management, with a strong understanding of community health initiatives.
  • Proven ability to design and implement programs that improve health outcomes.
  • Experience working with data analytics tools and platforms to track and report on health metrics.
  • Strong project management skills, with the ability to manage multiple initiatives and prioritize effectively.
  • Excellent communication and interpersonal skills for effective collaboration with diverse stakeholders.
