Job summary

CalOptima Health is seeking a X - As Needed - Program Manager, PHM and Clinical Operations to drive design, development and execution of Population Health Management initiatives. You will coordinate PHM activities with Clinical Operations, regulatory bodies and external partners to close member care gaps and improve engagement.

The role requires a bachelor's degree in a relevant field and ~3 years of program development experience, with partial telework and required in-office presence in Orange,

Qualifications

• Bachelor's degree in public health, health administration or related field required; 3+ years in program development and implementation.
• Equivalent combination of education and experience may qualify.

Responsibilities

• Lead PHM program design, development, implementation and support aligned with DHCS PHM strategy.
• Coordinate, implement and complete PHM tasks; oversee NCQA alignment and regulatory reporting.
• Develop workflows, KPI dashboards and executive reports; monitor progress and risks.
