Program Manager II, Clinic Partnerships

This position is remote in Washington State only.

Position Purpose:

The Program Manager, Clinic Partnerships, deploys improvement initiatives and facilitates best practice sharing within Community Health Plan of Washington's network of community health centers (CHCs). The role involves providing structured quality improvement support (practice coaching) and facilitating network learning opportunities (webinars, newsletters, roundtables) to support primary care transformation and enhance health outcomes for members.

Principal Duties:
  • Community Health Center Engagement:
  • - Build relationships with CHC contacts, manage site visits and calls, provide data on performance metrics, analyze data for improvements, and promote CHPW initiatives.
  • - Develop annual quality improvement work plans with each CHC and guide them towards completing these plans.
  • - Serve as the primary contact for quality issues, liaising with other departments like IT, Marketing, and Provider Relations.
  • Community Health Center Learning and Communications:
  • - Identify opportunities for shared learning and facilitate peer connections on quality measures.
  • - Collaborate on webinars, newsletters, and other communications.
  • - Organize in-person Quality Improvement Roundtable events.
  • Community Health Center Resources Development:
  • - Create written and electronic resources such as toolkits, program overviews, and performance reports to support quality improvement and incentive programs.
  • Requires up to 25% in-state travel; some overnight travel may be needed.
  • Other duties as assigned.
