Job summary

Qualifications

• A minimum of five years' experience working in mental health/ID field in administrative/management, supervision and direct service roles.
• Valid driver's license with an acceptable driving record.
• Knowledge of Federal, State and Local guidelines, including person-centered practice and Medicaid waivers.
• Bar to barrier crimes: not eligible if convicted of barrier crime per Virginia law.

Responsibilities

• Manage day-to-day client chart as per program policy.
• Coordinate rehabilitative and behavioral health services for individuals with disabilities.
• Develop assessments, discharge planning, and individual service plans with leadership input.
• Facilitate new employee orientation for Direct Support Professionals.
• Submit serious incident reports and ensure internal investigations are completed.
