SCHEDULE: Mon 6a-9a (DS)/9a-2p (Admin), Tues 8h FLEX (Admin), Wed 1:30p-11:30p (DS), Thurs 6h FLEX (Admin), Fri 1p-9p (DS)

Responsibilities
  • Direct Support Responsibilities: Manage groups of individuals in the program and community; provide safe transportation when outside of the residence; assist with People Supported personal care and activities of daily living; perform housework; implement skill building programs and behavior support plans.
  • Provides therapeutic support: Deliver an intensively therapeutic program designed to enhance community acceptability and increase the People Supported’ level of independence, including self‑care, community integration, and all aspects of activities of daily living, by scheduling the events of the People’s day and conducting the programs in the home and community.
  • Schedules: Provide current program/staff schedule as requested by the Director of Support Services.
  • Communication: Coordinate communication between the program(s) and the interdisciplinary team to provide members with information and access necessary for adequate care.
  • Financial responsibilities: Coordinate expenditures, maintain a running budget on all appropriate budget lines with the Assistant Controller, and provide a Direct Care Expense Report (DCER) according to established timelines.
  • Meet as needed with members of the Administrative Team.
  • Attend all scheduled Program Manager meetings, general staff meetings, and clinical meetings and IP meetings.
  • Conduct directly or through delegation monthly house and consumer meetings and quarterly staff meetings.
  • Ensure implementation of all consumer treatment and behavior plans, teaching strategies, health and safety protocols, document as directed, and report regularly to the IDT.
  • Provide for the health and well‑being of the People Supported by assisting them in day‑to their environment, daily hygiene and clothing care, and by keeping records of social and medical information for each Person Supported.
  • Preserve the rights of each Person Supported and his or her family by maintaining confidentiality of all Peoples’ PHI under HIPAA guidelines, employing restrictive procedures only when specified in a Person’s treatment plan, and following the ethical standards of Journey Found.
  • Supervise all direct care staff in assigned programs regularly.
  • Adhere to all safety practices and maintain a safe environment in assigned program(s).
  • Cooperate with special requirements made by the Chief Executive Officer or his/her designee.
  • Provide assistance to program staff in developing and implementing productive house management routines, schedules, and procedures to meet Peoples’ Supported IP goals and objectives.
  • Planning and development responsibilities.
  • Performance evaluation.
  • On‑call responsibilities.
  • Privacy of health information: adhere to Journey Found policy & procedures, protecting the privacy and security of People Supported health information.
  • Electronic media security: maintain the security of electronic media and return media to supervisor when appropriate.
  • Other responsibilities as assigned by supervisor.
Qualifications & Requirements

Computer literacy required; fluency in Word, Excel & Outlook.

Minimum of two years experience in the Human Services field.

Behavioral management experience and/or formal training.

BA/BS in Human Services, ABA, or related field preferred.

Leadership experience preferred.

Ability to communicate, both orally and in writing.

Must be free from communicable disease.

Must have at least three years of driving experience with a valid driver’s license.

Essential Physical Requirements

The ability to move independently throughout a wide range of environments, some of which may not be wheelchair or otherwise handicapped accessible, is required. Independent transportation to locations that may not be served by public transportation may be necessary.
