Job summary

Journey Found is seeking a on-site care professional for evening shifts (Mon-Fri, 4p-9p) in Windsor, CT. The role involves assisting individuals with daily living activities, transportation, medication administration, and coordinating activities and appointments with clinical staff.

Requirements include a high school diploma and driving experience; CPR/FA and MAP training are expected or provided. Benefit-rich employer with state-of-the-art training resources.

Qualifications

• High School Diploma or equivalent.
• 3 years driving experience with a valid driver’s license.
• PMT, CPR, FA and Medication Administration Program (MAP) training may be required.

Responsibilities

• Supporting individuals with activities of daily living (getting dressed, cooking, cleaning, bathing, toileting).
• Transporting people to appointments or activities.
• Organizing activities and community outings.
• Medication administration as directed.
• Attending doctors’ appointments with individuals.
• Documenting program data for tracking and reporting.
• Teaching and engaging individuals in routines.
• Implementing good daily routines and structure.
• Assisting in decision-making processes.
• Grocery shopping and running errands as needed.
• Deescalating situations and managing behaviors.
• Working as part of a collaborative team.
• Communicating professionally with Nurses, Clinical Staff, Directors and Supervisors

Requirements

• Must have at least a High School Diploma or its equivalent
• Must have at least 3 years driving experience with a valid driver’s license
• Behavioral Management Experience and/or formal training
• Ability to communicate, both orally and in writing
• Will be required to be PMT, CPR, FA and complete DDS Medication Administration Program (MAP)
• Must be free from communicable disease
