Program Facilitator – Day Services – Silsbee

Annual Salary: $39,521 - $43,186

Job Purpose

Provides services to adults with intellectual and developmental disabilities. The facilitator is responsible for overall program structure, supervision of assigned units and programs, direct care services, unit classroom management, staff documentation, supply inventory, performance evaluations, and data collection for program evaluations. Utilizes Center tools and techniques, person‑directed planning, and operates within the guidelines of the Performance Contract, HCS, and TxHmL Principles and Billing Guidelines.

Education and Experience Required
  • High school diploma or GED plus four (4) years of experience in a related field.
  • Bachelor's degree may substitute for three years of experience.
  • Experience must include effective oral and written communication skills.
  • Preference for applicants with at least two years of supervisory experience, experience working with individuals who have severe or profound intellectual deficits, severe physical disabilities, or are on the Autism Spectrum, and knowledge of behavior‑therapy techniques and SAMA.

Driving Required

Yes – a current State of Texas driver license is required (or a current license from another state if the applicant resides there).

Knowledge, Skills & Abilities
  • Effective oral and written communication skills including good spelling and grammar.
  • Ability to write progress notes, assessments, brief mental status exams, memos, letters, and various reports with acceptable spelling and grammar.
  • Computer proficiency with Microsoft Office and Google applications.
  • Knowledge of behavior‑therapy techniques for individuals with IDD, physical disabilities, and/or on the Autism Spectrum.
  • Knowledgeable in SAMA.
  • Maintains and demonstrates principles of confidentiality.
Other Requirements or Conditions
  • Interact with consumers, referral sources, outside agencies, and family members as needed.
  • Must have liability insurance if operating a personal vehicle for Center business.
  • Complete all required training in the month in which they are due.
  • May be required to provide on‑call duties.
  • Availability of a personal vehicle for business use may be required.
  • Work will take place in office, community settings, and clients’ homes, including shelters and temporary housing.
  • Must submit to and pass a pre‑employment drug test.
  • Must successfully pass all background screens required by the state.
  • Ability to input data into a computer, perform calculations, complete paperwork, and produce reports.
  • Must be physically able to conduct home visits, walk on uneven surfaces, climb stairs, and stand for long periods if required.
  • Implements Person‑Directed Planning by scheduling and coordinating services throughout the program area using quality‑improvement tools and techniques.
  • Provides exemplary direct care services as required by unit/center policies and procedures.
  • Assigns newly referred individuals to classroom and service staff at program entry.
  • Develops and coordinates special interest and seasonal activities on and off site.
  • Maintains inventory of all supplies and materials with 90% accuracy.
  • Prepares accurate requisitions for necessary materials to ensure timely classroom use.
  • Performs blind inventory as scheduled by the accounting department and prepares survey documentation for unusable items.
  • Assists co‑workers and staff as necessary to meet individual needs.
  • Demonstrates appropriate responsiveness to customer needs as evidenced by Consumer Satisfaction Ratings.
  • Apprises the Administrator of MR Support Services Programs of progress, accomplishments, needs, and concerns to provide recommendations.

  • Onboards employees including submission of requisitions, conducting interviews, and completing the hiring process per Human Resources guidelines.
  • Conducts performance evaluations of assigned staff on time and provides fair and meaningful feedback.
  • Schedules staff for mandatory training to ensure competence and development.
  • Monitors departmental and staff performance using established reporting processes and provides feedback and coaching.
  • Manages staff schedules, attendance, and punctuality, and reports staff time accurately.
  • Ensures staff are aware of and adhere to Center policies and procedures.
  • Observes direct care staff providing service at least once per year and documents through the Clinical Supervision Form.
