Job summary

Services for the UnderServed is seeking a Program Director to supervise all program staff regarding Case Management and ensure quality care for residents with mental health diagnoses, HIV/AIDS, and histories of substance use.

The position requires a Master’s degree, 3–5 years of supervisory experience, and preferred bilingual English/Spanish skills. This is an in-person, on-site role with a salary range of $70,000–$80,000 per year.

Responsibilities

• Supervise all program staff regarding Case Management and ensure quality care for residents.
• Ensure compliance with Federal, State and Local laws and regulations.
• Develop and implement individual client service plans with clinical staff, counseling team and client.
• Coordinate with annual Functional Assessment Review and report to committees.
• Maintain contact with other service agencies and coordinate client placements for treatment and activities.
• Coordinate with Intake Coordinator on orientation and transition of new residents.
• Participate in discharge planning and provide emergency coverage as needed.
• Represent the agency with community groups and other organizations.
• Prepare reports for leadership and maintain documentation of supervisory meetings.
• Work flexible shifts including weekends as assigned.

Position Overview

• SCOPE OF ROLE :

The Program Director will provide supervision of all program staff with regard to Case Management to ensure quality care for residents with mental health diagnosis, HIV/AIDS, and histories of substance use.

• ESSENTIAL DUTIES & RESPONSIBILITIES:
• Familiar with Federal, State and Local laws and regulations as they apply to the community residence program and agency policies
• Direct supervision of all program staff with regard to Case Management
• Development and implementation of individual client service plans in conjunction with clinical staff, counseling team and client
• The Program Director is responsible for ensuring that contracts with clients reflect goals of service plans for assisting counselors to develop intervention goals and activities which support the service plan
• Coordinates Annual Functional Assessment Review for each client, submitting report and recommendations to Utilization Review Committee.
• Responsible for maintaining contact with other service agencies and insuring client placement for treatment and program activities (may include volunteer job placement)
• Responsible for coordination with Intake Coordinator of orientation and transitioning into program of all new residents
• Participates in discharge planning
• Provides emergency and relief coverage as necessary
• May represent agency with community groups and agencies
• Prepare report s for Assistant Deputy Director of Mental Health Services and Executive Director, including any problems, and proposals for changes in policies or program design
• Provides supervisory meetings with all staff and maintains documentation of these meetings
• Ability to work flexible shifts including weekends as assigned
• Other duties as assigned including any problems, and proposals for changes in policies or program design
