Job Description

The Lifestyle Medicine (LM) Program is designed to help patients living with chronic cardiometabolic conditions adopt healthy lifestyle changes across the pillars of nutrition, physical activity, sleep, stress management, social connections, and avoidance of risky substances. Clinical criteria for referral to this program include atherosclerotic cardiovascular disease, hypertension, prediabetes, type 2 diabetes, and/or health concerns related to obesity. The Program Coordinator will oversee the day-to-day operations and data management of the LM Program.

Summary Of Duties And Responsibilities

  • Convert information gathered into specific details including data sources, data and user types, interface components, interface navigation needs, reporting needs and administrative system needs.
  • Document changes to baseline requirements through standard change control process, developing and implementing strategies to improve program performance and operations (i.e., reduce patient cycle time, improve access, decrease wait time, improve patient experience).
  • Analyze and document business requirements and processes; communicate requirements to technical personnel through the construction of basic conceptual data, process models and flowcharts, and technical specifications.
  • Coordinate key administrative initiatives including new hire placement, room assignments, and access for software systems as needed (e.g., Epic, Outlook, Microsoft Network).
  • Plan, develop, and may conduct trainings to ensure clerical staff consistently meet patient experience standard including tone, body language and attitude, and display caring actions even in stressful situations
  • Define external interfaces, constraints, quality issues and other non-functional requirement
  • Plan, coordinate, and support unit acceptance testing and subsequent deployments, in coordination with other team leaders
  • Complete annual regulatory requirements within established timeframes.
  • Perform data entry/management including generating reports and maintaining a HIPAA-compliant database of program patients and their follow-up visits
  • Perform end user and application support functions including problem solving and resolution of application function defects.
  • Provide training to and serve as a mentor for junior clinical business analysts and/or other appropriate project team members; manage and evaluate work products for assigned team members
  • Manage intake of patients to program, including outreach to prospective patients to provide overview of program, schedule initial visits, and track program enrollments
  • Assist with scheduling program appointments as needed (remote or in-person visits to physicians, nurse practitioner, dietitian, health coach, psychologist).
  • Provide training to and serve as an advisor to the departments that intersect with program - eg, Finance, and Nursing. Manage and evaluate work products in alignment with partnership objectives.
  • May assist with requests for authorization for dietitian visits and other services.
  • Coordinate logistics of program activities, including group visits (video and/or in-person), cooking demonstrations, program staff meetings, produce deliveries, and Health Bucks distribution.
  • Interview junior level analysts and/or other appropriate project team members and be able to supervise - i.e., AmeriCorps support interns, clerical staff and other functions as available.
  • Identify opportunities for process improvements and provide analysis and propose change and communicate strategies
  • All other duties as assigned

Minimum Qualifications

Educational and/or experience requirements will vary requiring diverse kinds and levels depending on the content, complexity and variety of project. Compensation levels will correspond to the requirements developed for the designated project.

In No Event Will The Acceptable Minimum Be Below

  • A Baccalaureate Degree from an accredited college or university in a specialization discipline consistent with project content; and,
  • Four years of substitutable and relevant health care experience related to the objectives of the project; or,
  • A satisfactory equivalent combination of experience, training and education.
