Fraser is seeking a Program Coordinator to join our Community Living team in St. Paul, MN. In this leadership role, you will supervise staff, guide individuals served, and ensure services align with 245D regulations and Fraser policies.

You’ll oversee one apartment complex within our five Supportive Living settings and help create a positive team culture. You’ll coordinate intakes, discharges, and services with families and providers, develop staff training, and ensure accurate documentation and

Qualifications

• Experience with individuals with mental health needs.
• Supervisory or program coordination experience preferred.
• Valid driver’s license and ability to meet 245D qualification requirements.

Responsabilités

• Provide leadership, supervision, coaching, and support to Community Living staff.
• Ensure individuals receive quality services in accordance with plans and Fraser policies.
• Coordinate intakes, discharges, and services with families and healthcare providers.
• Develop and provide staff training and support professional growth.
• Support staff with problem-solving, conflict resolution, and day-to-day questions.
• Ensure documentation, billing, payroll, and records are completed accurately and on time.
• Maintain compliance with HIPAA, 245D, and Fraser policies and procedures.
• Respond to urgent needs and participate in on-call rotation.
