Open Circle of Apple Valley is hiring a Program Coordinator to join our adult day center team. The role involves supporting members with purposeful activities, implementing treatment plans, and coordinating outings; hours 8:00 am to 4:30 pm, Monday through Friday, with no evenings or weekends. Ideal candidates have a High School Diploma, healthcare or social service training, or a related Bachelor's degree; CTRS or COTA is preferred.

Location: Apple Valley, MN. Pay: $19 per hour.

Qualifications

• High School Diploma or equivalent.
• Formal training/licensure in health care or social services or related Bachelor's degree, or equivalent experience (preferred).
• CTRS or COTA certification preferred.
• Experience with elderly and/or disabled persons.

Responsibilities

• Help members reach and maintain highest level of performance and independence.
• Assist with individual care by implementing treatment plans (ADLs).
• Prepare, implement, and evaluate daily programs (arts, games, reminiscence).
• Assist with planning community outings and coordinate transportation.
• Maintain courteous, respectful interactions with members, families, and staff.
• Update social workers and case managers on progress and changes.
• Perform additional tasks as needed.

As a Program Coordinator at Open Circle of Apple Valley, you will work with a dedicated team to assist members with purposeful activities such as creative arts, brain games, reminiscence and community engagement. Our ideal candidate is passionate about working with mature adults (including those living with dementia), thrives in a positive work environment, and has good critical thinking skills. Those holding CTRS or COTA credentials are highly encouraged to apply.

Position Type: Full-Time

Hours: 8:00 am - 4:30 pm

Pay: $19 per hour

Program Coordinator Responsibilities

• Help each member reach and maintain the highest level of performance and independence
• Assist with individual care by implementing treatment plans. This includes ADLs such as toileting, personal hygiene, etc.
• Prepare, implement, clean up, and evaluate assigned daily programs for members. Daily programs include activities such as table games, arts and crafts, word games/ trivia, music and more!
• Assist with planning and implementing all aspects of assigned community outings; schedule and coordinate transportation.
• Display an attitude of courtesy and respect for all members, families, and staff.
• Update social workers and case managers on members’ progress, needs, and/or changes of behaviors. A small caseload may be included with the position.
• Perform additional tasks as needed.

Program Coordinator Qualifications

• High School Diploma or equivalent.
• Formal training and/or licensure in the health care or social service field or Bachelor’s degree in human services, recreational therapy or related field or equivalent experience (preferred).
• Caregiving (HHA or PCA) experience preferred.CTRS or COTA certification preferred.Experience with elderly and/or disabled persons.
