Job summary

Sunrise Community, Inc. is seeking a Program Coordinator in Greeneville, TN to support individuals with intellectual and developmental disabilities. You will develop, implement, and coordinate individualized program plans, and ensure training aligns with needs and regulatory requirements.

The role requires a valid driver's license, strong documentation skills, and prior experience in related fields. This is a full-time, on-site position with comprehensive benefits and a drug-free workplace

Qualifications

• Must successfully complete the required pre-employment Background and Drug Screenings
• Must possess a valid Driver's License with all necessary endorsements and have a minimum of 3 years of driving experience
• Must possess advanced computer skills
• Must possess a High School Diploma or equivalent, bachelor’s degree preferred
• Experience working in the field of intellectual/developmental disabilities preferred
• Minimum one year of experience in writing program plans, case notes or other relevant documentation required

Responsibilities

• Develop, implement and coordinate individual’s program plans
• Develop, implement and monitor individualized training programs to meet identified priority training needs
• Ensure all plans are person centered and reflect the individual’s hopes and dreams through active participation wherever possible
• Ensure all documentation is accurate and complete according to regulatory requirements
• Communicate with all team members, including family, guardians and support coordinators to ensure that the individual’s needs are being addressed
• Performs other duties as assigned
