Job summary

Community Options is seeking a Program Coordinator for Residential Services to work directly with individuals, coordinating housing, finances, education, employment, health care, recreation and more. You will develop and review each individual's Overall Service Plan (OSP) to promote independence and community involvement.

Key duties include assisting individuals in achieving personal goals, ensuring compliance with standards, and coordinating services with a person-centered focus.

Qualifications

  • Bachelor’s degree required, in Human Service, Developmental Disabilities, Social Work, Psychology, Behavioral Sciences, or related field preferred AND one year of DD experience or related field.
  • CPR/First Aid certification required or must complete and certify within 30 days of employment.
  • A valid driver’s license with acceptable driving record and reliable transportation may be required.
  • Criminal background check, drug screening, and pre-employment approval required.
  • DD certification may be required for some bachelor degrees outside listed fields.
