Job summary

Mentor Community Services, part of the Sevita family, seeks a Program Clinical Coordinator for our Early Intervention program serving infants to toddlers in the San Bernardino area. This full-time role involves coordinating services, oversight of care, and collaboration with families and external case managers.

Responsibilities include developing and reviewing plans, ensuring quality and regulatory compliance, and guiding Early Intervention Specialists.

Qualifications

• Bachelor's degree in human services or related field.
• One year of work experience with individuals with IDD or related groups.
• Valid CA Driver's license and at least one year as a licensed driver.
• Reliable vehicle with current registration and insurance.
• Self-motivated, detail-oriented, able to multi-task and solve problems.
• Strong analytical and communication skills with team rapport.
• Flexible availability.

Program Clinical Coordinator - Early Intervention Program

This role works in our Early Intervention program working with children who have been diagnosed with an intellectual or developmental disability. The current opening is working with the infant-toddler age range, which includes children between ages 0-3 years old.

Annual Salary: $47,278 plus mileage reimbursement, overtime eligible

Full-Time 40 hours/week

• Coordinate the development, review, and implementation of services, case management, behavior intervention plans, and treatments provided to individuals served.
• Oversee all services provided to individuals and their families, monitor quality of care delivered, ensure compliance with contractual terms, and monitor regulatory program compliance.

• Use ABA and behavioral skills and knowledge to be extra level of support to challenging cases and support the Early Intervention Specialists. Provide supervision, training, and guidance to Early Intervention Specialists on special techniques and skills in working with parents and children.

Qualifications

• Bachelor's degree in human services or related field required (Acceptable majors include Psychology, Sociology, Child/Human Development, etc.)
• One year of work-related experience working with individuals with intellectual and developmental disabilities, brain injury, youth placed at-risk, etc. - We are primarily looking for candidates who have experience working with children and their parents/guardians for this specific role.
• Valid CA Driver's license; also must have at least one year as a licensed driver (in any US state)
• Reliable vehicle with current registration and insurance
• Self-motivated and detail-oriented with ability to multi-task and ability to exercise sound judgment and independently problem-solve.
• Strong analytical and communication skills with ability to build rapport with a team.
• Flexible availability desired.
