Job summary

Sevita is hiring a Case Management Supervisor for our Early Intervention program in the San Bernardino area. You will supervise the therapeutic case management team and oversee infant-toddler cases (ages 0-3) with developmental disabilities, coordinating ISPs, treatment plans, and staff performance.

You’ll work a full-time, Mon-Fri daytime schedule, with occasional on-call coverage. The role offers mileage reimbursement and overtime eligibility in a mission-driven environment.

Qualifications

  • Bachelor’s degree required in a health/social services field
  • 2–4 years of experience in human services, with at least 1 year in case management
  • Direct experience supporting children and families in a therapeutic setting preferred
  • Valid CA driver's license; licensed driver for at least 1 year
  • Reliable vehicle with current registration and insurance
  • Self-motivated, detail-oriented, able to multi-task and solve problems independently
  • Strong analytical and communication skills with ability to build rapport within a team
  • Availability during typical business hours (Mon-Fri) with on-call flexibility as needed

Responsibilities

  • Supervise cases within the assigned management team for infant-toddler children with developmental disabilities; provide direction and support weekly
  • Oversee performance evaluations, scheduling, orientation, training, and HR actions including hires, transfers, promotions, and salary changes
  • Assign cases to Specialists and Coordinators; review treatment plans and ensure data accuracy and compliance
  • Interview individuals served, collect medical history and administer diagnostics; document results
  • Assist in development and implementation of Individual Service Plans (ISP) with annual and ongoing meetings
  • Consult with treatment team in changes to the plan and arrange additional support as needed
  • Plan and administer therapeutic treatments based on clinical competency
  • Refer individuals to supportive services as required
  • Provide on-call coverage for caseloads and crisis intervention as needed
  • Conduct supervisory home visits and counsel parents/guardians

Case Management Supervisor - Early Intervention Program

Annual Salary: $49,920 plus mileage reimbursement, overtime eligible

Position Status : Full-time (40 hours/week)

Typical Schedule : Mon-Fri daytime - may include some light on-call responsibilities after hours and weekends

As a Case Management Supervisor for our Early Intervention program in the San Bernardino area, you will supervise cases and manage/supervise the therapeutic case management team for the program.

This position works with infants to toddlers (ages 0-3) who have been diagnosed with an intellectual or developmental disability, as well as the parents/guardians of the children in the program.

  • Supervise cases within the assigned management team for children (infant-toddler age) with intellectual or development disabilities; provide direction, support, and counsel to team members every week.
  • Manage and supervise case management team, including performance evaluations, scheduling, orientation, and training as well as making recommendations on decisions for employee hires, transfers, promotions, salary changes, discipline, terminations, and similar actions. Resolve employee problems within position responsibilities.
  • Determine caseloads and assign cases to Specialists and Coordinators; review treatment plans to ensure completeness and accuracy; track all necessary data related to case management performance targets and timeliness of reports; maintain internal and external file compliance.
  • Interview individuals served to obtain medical history data and other pertinent information; conduct intake and administer diagnostic tests to identify behavioral and personality traits; document results.
  • Assist in the development and implementation of the Individual Service Plans (ISP) including annual, ongoing meetings.
  • Consult with a treatment team, including a medical doctor and other specialists, in decisions related to changes in a treatment plan, amend the plan as directed, and arrange additional support as necessary.
  • Plan, arrange, and/or administer therapeutic treatment such as behavior modification, developmental milestones, social skills, and so on based on clinical competency.
  • Refer individuals to supportive services to supplement treatment and counseling as required.
  • Provide on-call coverage for caseload and participate in rotational emergency coverage and crisis intervention support, as needed.
  • Conduct supervisory home visits and counsel parents/guardians of children in program, as needed.
  • Ensure that initial and ongoing RBWO waivers are completed and submitted promptly, and that SCORE is updated and accurate at all times if required by state.

Qualifications:

  • Bachelor’s Degree in Major/field related to Psychology, Sociology, Child or Human Development, or other health or human services majors.
  • Please note: We are unable to make exceptions on education - must have completed at least a Bachelor's level degree
  • Two to four years of experience in human services INCLUDING at least one year of experience as a Case Manager (or similar title, as long as primary duties of position are case management related)
  • Must have experience directly supporting children and families in a professional role, preferably in a therapeutic capacity
  • Valid CA Driver's license; also must have been a licensed driver for at least one year (in any US state)
  • Reliable vehicle with current registration and insurance
  • Self-motivated and detail-oriented with ability to multi-task and ability to exercise sound judgment and independently problem-solve.
  • Strong analytical and communication skills with ability to build rapport with a team.
  • Availability during typical business hours (Mon-Fri daytime) plus some flexibility to provide on-call support after hours and/or on weekends as needed
