Program Clinical Coordinator (Case Management)

Program Name: California MENTOR Family Home Agency
Office Location: Alhambra
Program Area: East LA, Whittier, Pico Rivera, Montebello (travel required)
Pay: $45,800 annual salary (overtime eligible)

Responsibilities
  • Coordinate development, review, and implementation of services, case management, behavior intervention plans, and treatments for individuals served.
  • Oversee all services provided to individuals and their families, monitor quality of care delivered, ensure compliance with contractual terms, and ensure regulatory program compliance.
  • Assist in the development of quality services and engaging activities that meet individuals’ needs.
  • Build and maintain relationships with families and external case managers.
Qualifications
  • Bachelor’s degree in human services or a related field.
  • One year of work‑related experience with individuals with intellectual and developmental disabilities.
  • Valid California driver’s license and reliable vehicle with proof of registration and insurance.
  • Reliable, responsible, compassionate approach and commitment to quality.
  • On‑call availability after hours and on weekends.

Applicants without a bachelor’s degree will be considered if:

  • High school diploma or GED.
  • At least two years of experience in a similar program serving individuals with developmental disabilities.
