Sevita’s MENTOR Adult Day Health in Brockton, MA seeks a Program Services Case Coordinator to support participants by coordinating services, maintaining documentation, assisting with admissions, and ensuring continuity of care across teams and providers.

You will help participants and families understand program goals, track medical documentation, and coordinate internal appointments while promoting a person-centered approach to care within a compassionate team environment.

Qualifications

• Bachelor’s degree in a social services–related field is required.
• Two years of experience in medical and/or rehabilitation settings preferred.
• Experience with individuals with disabilities preferred.
• Strong organizational skills and attention to detail.
• Excellent written and verbal communication skills.
• Compassionate, reliable, and responsible approach to supporting participants.
• Commitment to the organization’s mission and values.

Responsibilities

• Coordinate and oversee individualized participant programs to support clinical, social, and community-integration needs.
• Assist participants and families in understanding program goals, services, and processes; serve as primary contact and provide resources.
• Review evaluations, progress notes, and discharge documentation to ensure accuracy and coordinated care goals.
• Support discharge planning and follow-up coordination.
• Coordinate internal medical appointments and track required annual medical documentation.
• Communicate with healthcare providers, community resources, and external partners to promote continuity of care.

MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

Program Case Coordinator – B1 Adult Day Health Center

As the Program Services Case Coordinator, you will play an essential role in supporting Adult Day Health participants by coordinating services, maintaining accurate documentation, assisting with admissions, and ensuring continuity of care across internal teams and community providers. This position offers the opportunity to make a meaningful impact while helping participants achieve their clinical, social, and community goals.
