Job summary

The Children’s Home is seeking a Program Assistant for the Youth ACT program on-site in New York. You will work with a collaborative team to support youth ages 10-21, helping prevent hospitalizations and out-of-home placements through administrative and care coordination tasks.

Responsibilities include maintaining EHR logs, coordinating team communications, and attending model-specific training to ensure consistent, high-quality services.

Qualifications

  • High School Diploma required.
  • Associates Degree in related field preferred.
  • 1 year of administrative assistant experience.
  • Driver's License and maintain insurability required.

Responsibilities

  • Maintain accurate and up-to-date logs and case records in the agency EHR.
  • Serve as frontline representative for Youth ACT participants, families and supports.
  • Support team with care/case coordination.
  • Attend daily team meetings and take notes for the team.
  • Attend specialized Youth ACT model training.
  • Take initiative and work independently.
  • Support team with tasks like calls, scheduling, calendar syncing, and errands.
  • Perform reception and other office duties as needed.
  • Complete quality call-backs to ensure services align with Youth ACT model.

Position Summary:

Make a difference in the life of a young person as a Program Assistant in the Youth ACT (Assertive Community Treatment) program. Work directly with a team that helps children ages 10-21 avoid hospitalizations and out-of-home placements. The team will work to provide skills and support to meet the needs of youth and families to overcome challenges. The program assistant serves as a non-clinical staff that uses a range of administrative assistant skills to keep the team organized and focused on providing high-quality care.

Responsibilities
  • Maintaining accurate and up to date logs and case records in the agency Electronic Health Record (EHR)
  • Serve as a frontline representative for Youth ACT participants, families, and supports, via phone, email, and in person communication
  • Support team with care/case coordination
  • Remain engaged and informed by attending daily team meetings and take notes of the meeting for the team
  • Attend specialized training in the Youth ACT model
  • Take initiative, work independently
  • Support the team in needed tasks which could include, placing calls, scheduling appointments, syncing calendars, running errands, etc...
  • Performing reception and other office duties that are needed
  • Completing quality call backs to ensure services provided are consistent with Youth ACT model
Requirements
Education:
  • High School Diploma required
  • Associates Degree in related field preferred
Experience
  • 1 year of administrative assistant service
  • Driver's License and ability to maintain insurability throughout employment required
